Should you choose to designate EvergreenHealth Hospice Services to receive a donation in memory of a loved one, we suggest the following verbiage in the obituary: “Memorial gifts can be made to the EvergreenHealth Foundation, 12040 NE 128th St, MS 5, Kirkland, WA, 98034-3013.”
Located on the grounds surrounding the Gene & Irene Wockner Hospice Center, the Commemorative Gardens offer a wonderful space for remembrance and celebration of a life. The Rose and Alpine Gardens serve as a lasting tribute to the people whose lives have been touched by the Hospice Care Center or with Hospice in-home services. Proceeds from the gardens benefit EvergreenHealth Hospice Services.
Bricks, benches, and panels are installed every June, in time for the annual Rose Garden Ceremony in July.

A tribute gift is a wonderful way to celebrate a special occasion, express thanks, or recognize someone dear to you. For example, a gift could honor celebrations such as:
A Tribute Card is sent on your behalf to whom you indicate should be notified of the gift. (The amount of your gift will not be disclosed).
If you wish, you may designate your gift to support a specific EvergreenHealth program or service line. The Foundation can provide you with donation envelopes to distribute to family and friends. Your family will be notified of others’ donations as soon as a gift is received.
